What is Adult Family Living?

When it comes to serving an aging family member, we realize families are often the best caregivers.  That’s why we are pleased to provide a stipend, experienced care management and nursing support to family members or close friends who wish to care for their loved ones at home. Charter Oak Home Care’s adult family living services can guide you through the process.

Client Eligibility

  • Is at least 65+ and receiving benefits under Title 19 (Medicaid) 
  •  Lives in Connecticut
  •  Client and direct caregiver must reside in the same dwelling

As an approved State of Connecticut provider, we are pleased to come alongside and support you!

Once a referral is made, a professional will determine which of four levels of care is needed based on the complexity of care needed.  Upon being approved for the program, you receive a tax free stipend based upon the approved level of care.

As part of the program, you will receive

  • 24×7 support and case management
  • A tax free stipend with direct deposit available
  • Customized care plans
  • Committed Registered Nurses for support calls and visits
  • Home safety assessment
  • Educational training on caregiver best practices

  • Compensation is determined by the level of care as advised by the Nurse and Access Agency Case Manager
  • Other government programs such as ADC, and Meals on Wheels, Food Stamps and Medicaid will not be affected by AFL enrollment.
