How Do We Qualify for Adult Family Living (AFL)?
For The Care Recipient
Individual is 65 or older
On Medicaid or Title 19
Live in Connecticut
Need Help With Daily Activities
For The Caregiver
Must be 18yrs or older
Must be a relative or a friend
Must live with the person receiving care
Cannot be a spouse

How Do We Get Approved to Start?

A professional determines your eligibility. The eligibility timeline changes depending on when you apply so if you think you may be eligible start the process today.
Once your eligibility is determined and you are able to start the program we will work with you to get the program started as soon as possible.
Shortly after the program is started you receive payment.

Charter Oak Home Care
When it comes to serving an aging family member, we realize families are often the best caregivers. That’s why we are pleased to provide a stipend, experienced care management and nursing support to family members or close friends who wish to care for their loved ones at home. Our AFL Program is offered through the Connecticut Home Care Program for Elders and Title 19.
